COVID-19 in Zimbabwe: Trends, Dynamics and Implications in the Agricultural, Environmental and Water Sectors ; : 77-85, 2023.

ABSTRACT

The coronavirus pandemic has led to diminished income, which threatens nutrition security. Cash transfer programs increase the resilience of poor and vulnerable households by giving them an improved ability to obtain food and healthcare, which lead to nutrition security. However, the provision of cash does not necessarily translate to ideal behaviours, such as procuring nutritious foods. This study investigated the determinants of procurement of legumes and animal source foods amongst potential beneficiaries of a cash transfer project. We conducted a barrier analysis study in Chiredzi, Zimbabwe. Structured interviews were administered to 90 purposively sampled respondents (45 doers and 45 non-doers). The study investigated the four most common behavioural determinants, perceived self-efficacy, perceived social norms, perceived positive consequences and perceived negative consequences. Non-doers were 1.89 times more likely to state that receiving social assistance would make it easier to procure legumes and animal source foods for household consumption. Non-doers were 2.05 times more likely to state that the ability to barter their own possessions for legumes and animal source foods would make it easier to purchase the items for household consumption. Doers were 4.5 times more likely to report that relish comprising of animal source foods or legumes and pulses was tastier than other relishes. Doers were 12 times more likely to report that most people approve of purchasing animal source foods and legumes for household consumption. Doers were 3.23 times more likely to identify friends and relatives as the members of the community that approved of this behaviour. We identified three determinants of the procurement of legumes and animal-source foods, namely, perceived self-efficacy, social norms and perceived positive consequences. Cash transfer projects provide a first step to achieving nutrition security in the new normal but must be delivered with a context-specific behaviour change intervention. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2023.

ABSTRACT

Child hunger was prevalent during the COVID-19 pandemic, but the extent, determinants, and impact on pre-school children aged 6 months to 7 years old from Malaysian urban poor households are still unknown. This exploratory cross-sectional study was performed between July 2020 and January 2021 at the Lembah Subang People Housing Project, Petaling. The households' food security status was assessed using the previously validated Radimer/Cornell questionnaire, and the children's anthropometric measurements were taken. Food diversity score was assessed using the World Health Organization Infant and Young Children Feeding (under-2 children) or Food and Agriculture Organization Women's Dietary Diversity (2-year-old-and-above children) systems. Overall, 106 households were recruited. The prevalence of child hunger is 58.4% (95% CI: 50.0, 67.4). Significant differences were found in breastfeeding and sugar-sweetened beverage consumption between under-2 and ≥2-year-old children. There were no significant differences between child hunger and other food-insecure groups in weight-for-age, height-for-age, and weight-for-height z-scores. Only a higher dietary diversity score was significantly protective against child hunger after adjusting for maternal age, paternal employment status, and the number of household children (ORadjusted: 0.637 (95% CI: 0.443, 0.916), p = 0.015)). Proactive strategies are warranted to reduce child hunger during the COVID-19 pandemic by improving childhood dietary diversity.

ABSTRACT

Background: This study aims to assess factors associated with food security and dietary diversity among poor urban households of western Oromia, Ethiopia, after the outbreak of the Covid-19 pandemic. Method: A cross-sectional, community-based study was conducted in May to June 2021 with 361 poor urban households in the Horo Guduru Wollega zone, western Oromia, Ethiopia. A pre-tested structured questionnaire was used to collect primary data. Twenty-four hour reminder points were used to assess household dietary diversity, and household food security was assessed using the Household Food Insecurity Access Scale tool. Data were evaluated using the statistical software SPSS version 25.0. Results: This study showed a prevalence of food insecurity in households of 59.6%. The mean and standard deviation of household dietary diversity values were 4.19 ± 1.844. Family size (AOR = 8.5; 95% CI:3.295-21.92), monthly income (AOR = 3.52; 95% CI; 1.771-6.986), dietary diversity (AOR = 8.5; 95% CI; 3.92-18.59), knowledge (AOR = 3.0, 95% CI = 1.08-)8.347), attitude (AOR = 8.35, 95% CI:3.112-22.39) and practices against Covid-19 (AOR = 2.12; 95% CI:1.299-11.4) were factors significantly associated with food insecurity. Variables like educational status (AOR = 3.46; 95% CI:1.44-8.312), increased family size after the Covid-19 pandemic (AOR = 2.26; 95% CI:1.02-5.04), food security (AOR = 6.7; 95% CI:4.01-19.01), knowledge (AOR = 3.96; 95% CI:1.57-10.0), attitude (AOR = 3.9; 95% CI:1.75-8.82) and practices toward coronavirus (AOR = 2.23; 95% CI:2.18-23.95) were predictors significantly associated with dietary diversity. Conclusion: This study concluded that family size, monthly income, and dietary diversity were factors contributed to household food security. On the other hand, variables such as educational status, family size, and food security were highly relevant factors for dietary diversity after the outbreak of the Covid-19 pandemic. Knowledge, attitudes, and practices were also variables related to both household food security and dietary diversity. Therefore, immediate interventions such as nutrition-specific interventions can be suggested to address food insecurity and problems of inadequate food intake in poor urban households. In addition, governmental and non-governmental organizations should raise awareness and policies to support those at higher risk by developing affordable, sustainable and targeted social protection systems that ensure food security and adequate dietary intake at the household level.

The purpose of this manuscript is to describe household dietary diversity (HDDS) in Lusaka, Zambia between households with and without a child with a disability living in the same communities. Cross-sectional data were collected in three low-income compounds in September 2021. Participants included households with a child with a disability enrolled in Kusamala+, a community-based program, (n = 444) and a convenience sample of adults living in the same area without a child with a disability (n = 1027). The HDDS tool asked about food groups consumed in the past 24 h by people in the household. The responses were summed (yes = 1, no = 0), range 0-12. Individual dietary diversity scores (IDDSs) were calculated for children (0-8 items). Analysis included descriptive statistics and linear regression. Mean HDDS for the households with a child with a disability was 4.8 (SD 2.1) vs. 6.1 (SD = 2.2) among households without a child with a disability (p < 0.001). The individual score for children (IDDS) for households with children with disabilities was 2.6 (SD = 1.4) vs. 3.7 (SD = 1.6) for households without a child with a disability. Households with a child with a disability had a significantly lower HDDS and IDDS in unadjusted and adjusted models (p < 0.001). National policy must assure the most vulnerable populations, and often hidden, receive focused financial and food support.

ABSTRACT

This implementation research study sought to examine the impact of the COVID-19 pandemic on adolescent nutrition practices and related behaviors in Nairobi and Uasin Gishu Counties, Kenya. Eight focus group discussions (FGDs) were conducted with adolescents 10-19 years of age, in-depth interviews with 10 health facility providers, and a combination of FGDs (n-4) and key informant interviews with government stakeholder and implementing partners (n = 9). During the pandemic, adolescents tended to avoid commonly consumed junk foods, in favor of "immune boosting, protective" foods. Widespread unemployment and reductions in parental income rendered some food items such as meat, eggs, and fruits unaffordable for families of adolescents. Adolescents relayed experiences of skipping meals and reducing the amount and variety of foods consumed. Adolescents also described employing strategies such as working in the informal sector and selling personal items to support families financially, in response to rising food insecurity. School closures mandated during the pandemic likely contributed to reductions in overall physical activity. To improve the diets of adolescents, programs should build on the healthy mindset brought on by the pandemic, while strengthening, targeting, and improving access to social protection measures and agricultural initiatives for vulnerable families with adolescents to cushion them from rising food insecurity as an effect of COVID-19. Building practical adolescent life skills to encourage healthy nutrition actions will also be key to building forward from the COVID-19 pandemic in Kenya.

ABSTRACT

The policy measures of the government of Nigeria to restrain the spread of COVID-19, particularly in the initial three months (April - June 2020) led to significant disruptions to household livelihoods and food security. We investigate the effects of COVID-19 on food security and dietary diversity of households; focusing on the pathways through which income loss, endowments of wealth, social capital, and safety net programs moderate the severity of households' food security and dietary diversity. Primary data obtained from a telephone survey of 1,031 Nigerian households were analyzed using ordered logit and negative binomial models. Our results show that income losses due to the COVID-19 restrictive measures had pushed households into a more severe food insecurity and less diverse nutritional outcomes. Regarding wealth effects, livestock ownership significantly cushioned households from falling into a more severe food insecurity amid the pandemic. We found that because of the pandemic's indiscriminate effect across communities, the potential of social capital as an informal support mechanism might have been eroded to enable households to cope with shocks. Furthermore, safety net programs by the government and NGOs did not provide significant protection to households from falling into severe food insecurity and malnutrition amid the pandemic. We suggest three policy propositions - prioritize investment in local job creation to curb income loss; build the wealth base of households (e.g., land tenure security or livestock) to enhance resilience to shocks; and target safety nets and other social support programs spatially, temporally, and across social groups to enhance the effectiveness of such programs amid shocks.

BACKGROUND: Low birthweight (LBW) as well as early childhood stunting are risk factors for increased childhood morbidity in low-and middle-income countries (LMIC). The Covid 19 pandemic has exacerbated food insecurity and unemployment globally, prompting concerns for maternal and child health. OBJECTIVES: We used data from the great recession of 2008 to examine the relationship between household food security and other risk factors with LBW and stunting using a longitudinal sample of South African women and their offspring. METHODS: Food security indicators, alcohol use, blood pressure and other characteristics were examined in relation to LBW (≤ 2500 g), stunting (height for age ≤ 2SD) and severe stunting (height for age ≤ 3SD). Regression modelling with clustering at maternal ID level were employed to adjust for maternal characteristics and women who gave birth more than once during the reference period. RESULTS: Birthweight data were available for 1173 children and height for age 1216 children. The prevalence of LBW was 14.7% while stunting and severe stunting was 17.8% and 14.5%. Child hunger in the household, maternal hypertension and alcohol use were associated with low birthweight. Food expenditure below the Stats SA poverty line and low dietary diversity was associated with stunting and severe stunting respectively. Maternal height and low birthweight were associated with both stunting and severe stunting. CONCLUSIONS FOR PRACTICE: Interventions that can improve household food security and nutritional status during the periconceptional and antenatal period may reduce the prevalence of low birthweight and subsequent stunting in low- and middle-income countries.

ABSTRACT

Increasingly, rural households in developing countries are shopping for food online, and the COVID-19 pandemic has accelerated this trend. In parallel, dietary guidelines worldwide recommend eating a balanced and healthy diet. With this in mind, this study explores whether online food shopping boosts dietary diversity-defined as the number of distinct food groups consumed-among rural households in China. Because people choose to shop for food online, it is important to account for the self-selection bias inherent in online food shopping. Accordingly, we estimate the treatment effects of online food shopping on dietary diversity using the endogenous switching model with a count outcome variable. The results indicate that online food shopping increases dietary diversity by 7.34%. We also find that education, asset ownership, and knowing the government's dietary guidelines are the main factors driving rural households' decisions to shop for food online.

The outbreak of COVID-19, and the national-level lockdown to contain it, were expected to disrupt supply chains, lead to livelihood loss, and reduce household income. Studies anticipated a decline in food security in India, leading to a near famine-like situation. In this study, we examine the change in Dietary Score (number of food groups consumed out of a possible eight) and proportion of respondents complying with Minimum Dietary Diversity norms (consuming at least four food groups) among women aged 15-49 years and their youngest child (aged between 7 and 36 months) during the lockdown. The present study also analyses whether ownership of ration cards and contacts with the party in power locally helped the household to tide over the crisis. The data was collected through a two-phase primary survey undertaken in January-March 2020 (pre-lockdown period) and October-November 2020 (post-lockdown period). It was undertaken in six districts of Bihar, a state with a history of poor maternal and child health outcomes and dysfunctional delivery of health services. We find that dietary practices of women deteriorated, while that of children remained the same. The deterioration is less among households owning ration cards or having political contacts. The analysis suggests that, during pandemics or similar crisis periods, the need to supplement the supply of staple items through the Public Distribution System with a direct transfer of cash will allow households to maintain diversity in the consumption basket.

Background: The COVID-19 pandemic along with its confinement period boosted lifestyle modifications and impacted women and men differently which exacerbated existing gender inequalities. The main objective of this paper is to assess the gender-based differentials in food consumption patterns, dietary diversity and the determinants favoring weight change before and amid the COVID-19 pandemic among Arab men and women from 10 Arab countries. Methods: A cross-sectional study was conducted based on a convenience sample of 12,447 households' family members (mean age: 33.2 ± 12.9; 50.1% females) and information from participants aged 18 years and above was collected about periods before and during the pandemic. Results: Findings showed that, during the COVID-19 period, the dietary diversity, declined by 1.9% among females compared to males (0.4%) (p < 0.001) and by 1.5% among overweight participants (p < 0.001) compared to their counterparts. Conclusions: To conclude, gender-sensitive strategies and policies to address weight gain and dietary diversity during emergent shocks and pandemics are urgently needed in the region.

Introduction: The practice of infant and young child feeding (IYCF) is critical for a child's growth and development throughout the first two years of life. Poor feeding habits in early childhood contribute to malnutrition and child mortality in India. Aim and Objective: To assess the IYCF practices in children under the age of 2 years. Material and Methods: In rural Uttarakhand, India, a cross-sectional study was undertaken from March 2021 to May 2021. A probability proportional to size (PPS) method was used to select 400 children under the age of 2 years. The World Health Organization IYCF questionnaire, was used to collect house to house data. An appropriate statistical test was used for analysing the data. Results: According to the findings, 47.5 percent of babies under the age of six months were nursed within the first hour of delivery. About 73.9 percent of babies were exclusively breastfed. Approximately 22 percent infants were given pre-lacteal feeds and 20 percent were bottle fed. In addition to breast milk, half of babies aged 6 to 8 months had solid, semi-solid, or soft meals, however minimum acceptable diet was provided to only 33.5 percent children. Odds of male child who were bottle fed in the age group of 6-23 months were 2.02 times higher to that of female child. Also the odds of male child in the age group of 6-8 months to be introduced with solids, semi-solid, or soft food were 4.91 times higher to that of female child. Similarly, odds of male child received minimum dietary diversity (2.35), minimum meal frequency (1.82), and minimum acceptable diet (2.35) in the age group of 6-23 months were found to be higher to that of female child in the similar age group. Total of six mothers reported coronavirus disease (COVID) positive status and only two of them breastfed their babies using COVID appropriate behavior. Conclusion: Exclusive breastfeeding (EBF) has been practiced in more than two-thirds of children, but early breastfeeding is practised in less than half of children. Only one third children of more than six months of age are getting minimum acceptable diet.

PURPOSE: Maladaptive eating behaviors like food addiction (FA) are common among students, and orthorexia nervosa (ON) is being evaluated as a new condition among eating disorders (EDs). Moreover, dietary diversity (DD) is recognized as an important component of a healthy diet. Thus, the current study sought to examine the prevalence of ON and FA as well as the factors associated with ON, FA, and DD among university students in Bangladesh during the COVID-19 pandemic. METHODS: Four-thousand-and-seventy-six students were recruited and completed an online survey consisting of the Bratman Orthorexia test, the modified Yale Food Addiction Scale, and a questionnaire based on the Food and Agriculture Organizations' guidelines. Inferential statistics, bivariate and multivariable linear regression were used for analyzing data. RESULTS: ON and FA were observed in 1.7% and 7.5% of participants, respectively, with 43.8% exhibiting Health Fanatic eating attitude. The mean DD score was 5.96 (SD = 1.56). Students with ON consumed more legumes, nuts, seeds, and vegetables including dark green leafy vegetables whereas students with FA consumed more organ meats and eggs. Students who were older-aged, married, formerly smoked, had fitness goals, and had guilty feelings about violating food rules were more likely to have ON, whereas those who were female, were married, actively smoked, and were overweight and obese were more likely to have FA. CONCLUSIONS: The findings suggest that students from Bangladesh are at risk of FA, and less so for ON. These entities and low DD deserve more research attention in Bangladesh to increase awareness and ensure appropriate interventions.


Eating disorders, maladaptive eating behaviors, and low diversified diets are major public health issues amid the COVID-19 pandemic among university students. The current study aimed to address these issues by investigating Orthorexia Nervosa (ON), food addiction (FA), and dietary diversity. Four-thousand-and-seventy-six students completed an online questionnaire. It was found that being underweight was associated with ON. On the other hand, being overweight/obese was associated with having FA. Individuals who were former smokers and who had poor social interaction were significantly associated with having Orthorexia Nervosa. 1st-year university students are more likely to have FA compared to 5th-year/master's students. Active smokers were more likely to be food addicted. Students who had FA had less guilty feelings about violating food rules. Participants who had FA consumed more organ meats and eggs. Students who were from upper socio-economic status were more likely to have higher dietary diversity. Nutritional awareness programs focusing on DD and healthy eating habits should be implemented for students.

Wasting among children under-5 years remains a public health problem in Malawi, despite the quest to improve food availability through Farm Input Subsidy Program (FISP). As such, the study examined the link between FISP and child wasting. Using Malawi Integrated Household Panel Surveys for 2013, 2016, and 2019, two-stage least squares approach was employed to run a Cobb Douglas production function and a correlated Random Effects (CRE) Model to account for endogeneity challenges and an unbalanced panel dataset. The study hypothesized the role of FISP to dietary diversity at the household level on child wasting [weight-for-height (WHZ)]. Based on the analysis, the study found that household access to FISP coupons was not a stand-alone predictor for low wasting among children under-5 years. However, increased maize production due to FISP coupon access significantly correlated with lower wasting likelihood incidences at the household level. Worth to note, that in 2015/16, households that had accessed FISP coupons and were in the central region had higher wasting probabilities among the children under-5 years in Malawi compared to other counterparts panels. This implies challenges to addressing wasting among children under-5 years- which can be attributed to higher redemption costs of the FISP coupon. Therefore, the current study suggests that input subsidies can improve the reduction of wasting among children under-5 years through specific pathways, among them, increased maize production and considering appropriate targeted approaches to ensure households access the inputs for sustained food availability, which in turn enhances improved the children under-5 years health dividends in Malawi.

BACKGROUND: The computer-assisted telephone interview (CATI) has been used extensively during the COVID-19 pandemic, but the effects of respondent fatigue during these interviews on responses to questions about diet are unknown. OBJECTIVES: We designed an experiment that randomized the placement of a survey module on the dietary diversity of rural Ethiopian women and assessed whether responses were altered by placing this module earlier or later in a phone survey. METHODS: Two CATIs were implemented; in the second, women were randomly assigned to answer questions on diet diversity either earlier or later in the interview. Women's Dietary Diversity Scores were the primary outcome. Secondary outcomes were dichotomous measures of consumption from four or more and five or more food groups and consumption of food groups consumed frequently, often, and rarely. Impacts were assessed using a respondent fixed effects model. RESULTS: Delaying the food consumption module by 15 min in the interview led to an 8%-17% (P < 0.01) decrease in reported Dietary Diversity Scores, a 28% (P < 0.01) decrease in the number of women who consumed a minimum of four dietary groups, and a 40% (P < 0.01) and 11% (P < 0.01) decrease in the reporting of consumption of animal source foods and fruits and vegetables, respectively. Moving the food consumption module closer to the beginning of the interview increased the number of reported food groups consumed by older women, women with a below-median education level, and women in larger households. CONCLUSIONS: Our findings suggest that comparisons of descriptive statistics across studies and countries on metrics such as food security and dietary quality may be confounded by where these modules are placed in the interview, thus highlighting trade-offs between volume of information collected and data quality when designing CATI surveys.

The original aim was to determine the effect of egg consumption on infant growth in a low socioeconomic community in South Africa in a randomized controlled trial. Enrolment was, however, prematurely stopped due to COVID-19 lockdown regulations. The resultant small sample (egg group n = 70; control group n = 85) was followed up to assess the feasibility of egg consumption for eight months in terms of dietary intake, egg usage and perceived effects of lockdown on child feeding. Egg consumption remained low in the control group, <10% consumed egg ≥4 days/week at the follow-up points. In the egg group, egg was frequently consumed at midpoint (daily 87.1%, 4-6 days 8.1%) and endpoint (daily 53.1%, 4-6 days 21.9%). At endpoint, dietary intake of cholesterol and vitamin D was higher, and intake of niacin and vitamin B6 lower in the egg group compared to the control group. Dietary diversity was low, 36.2% of the egg group and 18.9% of the control group (p < 0.05) achieved minimum dietary diversity at endpoint. No babies developed egg allergy or sensitization, and adjusted regression analysis showed that frequency of egg intake was not related with the incidence or duration of allergy-related symptoms. This study showed that frequent egg consumption can contribute safely to complementary food for babies, especially in low- and middle-income countries.

Background: Dietary diversity (DD) is a key component of diet quality, and malnutrition due to poor diet quality leads to child morbidity and mortality. However, in Bangladesh, there is a lack of information on childhood DD (for children aged 6-59 mo) amid the coronavirus disease 2019 (COVID-19) pandemic. Objectives: The purpose of this study was to assess the minimum DD and its associated factors among children aged 6-59 mo during the COVID-19 pandemic in Bangladesh. Methods: A cross-sectional study was carried out in 6 districts of Bangladesh. A total of 1190 respondents were included using cluster random sampling. The Individual Dietary Diversity Score (IDDS) for children was used to assess the children's DD. Factors associated with DD of children were identified using a multilevel binary logistics regression model. Results: About 70% of the children aged 6-59 mo had minimum DD during the COVID-19 pandemic in Bangladesh. Children who belonged to slum areas [adjusted odds ratio (AOR): 0.45; 95% CI: 0.24, 0.83], family income 12,000-15,000 Bangladeshi taka (BDT) (AOR: 1.79; 95% CI: 1.06, 3.05) and >15,000 BDT (AOR: 2.59; 95% CI: 1.47, 4.57), mothers aged 26-30 y (AOR: 0.35; 95% CI: 0.20, 0.62) and >30 y (AOR: 0.43; 95% CI: 0.22, 0.85), respondents who had 2 children <5 y old (AOR: 0.43; 95% CI: 0.28, 0.66), and children aged 12-23 mo (AOR: 1.89; 95% CI: 1.14, 3.20) were significantly associated with DD among children aged 6-59 mo. Conclusions: The findings of this study highlight the need for food and nutrition-related intervention, particularly targeting mothers of younger age and with >2 children <5 y old, mothers from slum regions, and fathers who were unemployed, to improve children's DD practices.

ABSTRACT

The COVID-19 pandemic led to an economic crisis and health emergency, threatening energy efficiency consumption, sustainable food diversity, and households’ nutrition security. The literature documented that environmental threats can divert attention from renewable energy and food security challenges that affect humans’ environmental behaviors. The COVID-19 crisis has consistently influenced environmental behaviors, as it primarily decreased income and disrupted food systems worldwide. This study investigated the COVID-19 consequences on household income, sustainable food diversity, sustainable energy consumption, and nutritional security challenges. The study used a self-structured online survey due to non-pharmaceutical restrictions and collected data from 728 households. The investigators applied t-test and logit regression to analyze the data for drawing results. Descriptive statistics show that COVID-19 has adversely affected the income of more than two-thirds (67%) of households. The pandemic has influenced households’ food consumption, energy, and dietary patterns to safeguard their income. The t-test analysis indicated that households’ food diversity and energy consumption significantly declined during the pandemic, and households consumed low-diversified food to meet their dietary needs more than twofold compared to pre-pandemic levels. The results showed that all nutrient consumption remained considerably lower in the COVID-19. Cereals are the primary source of daily dietary needs, accounting for over two-thirds of total energy and half of the nutrient consumption amid COVID-19. The share of vegetables and fruits in household energy consumption dropped by 40 and 30%. Results exhibited that increasing monthly income was inversely associated with worsening food diversity and intake with energy efficiency. Compared with farmers and salaried employment, wage earners were 0.15 and 0.28 times more likely to experience a decline in consuming food diversity. Medium and large households were 1.95 times and 2.64 times more likely than small, to experience decreased food diversity consumption. Launching a nutrition-sensitive program will help minimize the COVID-19 impacts on energy consumption, food diversity, and nutritional security for low-income individuals. This survey relied on the recall ability of the households for the consumed quantities of food commodities, which may lack accuracy. Healthy diets promote immune functions and have been shown to reduce COVID-19 severity. In 2021, COVID-19 vaccines have become available to the general public. However, whether vaccination status could affect individual and populational health behaviors is unknown. This study aimed to investigate the impacts of vaccination status and confidence on dietary practices. An online survey was conducted in August 2021. We collected data on dietary intake, diversity and behaviors, vaccination status and confidence and socio-demographic characteristics. Among the 5107 responses received, a total of 4873 study participants were included in the final analysis. Most of our participants aged between 18 and 45 years and 82% of them were fully vaccinated against COVID-19. Household level dietary diversity was found to be higher among people who were fully vaccinated (ß = 0.321, 95%CI: 0.024 to 0.618) or who were more confident in the protectiveness of the vaccine (ß for tertile 3 comparing with lowest tertile = 0.544, 95%CI: 0.407, 0.682). Vaccination promoted the intake of seafood, but it was also positively associated with the consumption of sugar, preserved, fried and barbequed foods and reduced vegetable intake. Higher vaccination confidence was associated with increased consumption of seafood, bean, fruits and vegetables and reduced fat intake. Changes in dietary behaviors compared with early 2021 (when vaccination was not common) were observed and differed by vaccination status and confidence level. Conclusion: COVID-19 vaccination status and confidence had varied, and possibly negative, impacts on dietary intake and behaviors. Our results suggest that vaccination status and confidence might be significant influencing factors affecting people's health behaviors and highlight that healthy eating should be consistently promoted to prevent poor dietary practices during global health crisis.

ABSTRACT

Background: Fathers are key influencers of complementary feeding practices, but few studies in low- and middle-income countries have measured the effects of complementary feeding social and behavior change communication (SBCC) targeted at both fathers and mothers. Objectives: The aims of this study were to measure the effects of an SBCC intervention on children's dietary diversity (primary outcome) and other complementary feeding indicators, fathers' and mothers' complementary feeding knowledge, and fathers' support for complementary feeding (secondary outcomes). Methods: The 12-mo intervention in Kaduna State, Nigeria, engaged parents through community meetings, religious services, home visits from community health extension workers (CHEWs), mobile phone messages (fathers only), and mass media. Cross-sectional population-based surveys of cohabiting fathers and mothers with a child aged 6-23 mo were conducted, and regression models were used to compare results at baseline (n = 497) and endline (n = 495). Results: Children's minimum dietary diversity did not change from baseline to endline (62% to 65%, P = 0.441). Children's consumption of fish (36% to 44%, P = 0.012) and eggs (8% to 20%, P = 0.004) and minimum meal frequency (58% to 73%, P < 0.001) increased. Fathers' and mothers' knowledge of the timing of introduction of different foods and meal frequency improved. Fathers' support for child feeding by providing money for food increased (79% to 90%, P < 0.001). Fathers' and mothers' reported intervention exposure was low (11-26% across types of SBCC). Child feeding outcomes were not associated with fathers' exposure. Children's odds of both fish and egg consumption increased significantly with mothers' exposure to community meetings, religious services, home visits, and television spots, and children's odds of minimum meal frequency increased significantly with mothers' exposure to home visits. Conclusions: A multipronged SBCC intervention improved complementary feeding practices, fathers' and mothers' knowledge of complementary feeding, and fathers' support for complementary feeding, despite low levels of reported exposure, which may have been influenced by coronavirus disease 2019 (COVID-19) disruptions.
